These wall plaques were placed above certain doors in the asylum. This one is made of cast iron with the HBH insignia, HBH standing for Hertfordshire, Bedfordshire and Huntingdonshire. The plaques may have been decorative air vent covers. The example below measures approximately 9 inches x 7 inches.
